Certified Nursing Assistant - Day shift - Skilled Nursing Facility

Summary of Position:
Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, utilizing established policies and procedures, ensuring safe and effective care.
Services are provided based on the ages of the population servedadult through geriatric, 65 years or olderand in collaboration with the multidisciplinary care team.
Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team in meeting resident care goals.
The Certified Nursing Assistant SNF is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.
QualificationsEducation:
oCompletion of an accredited Nursing Assistant program
Experience:
oPrevious long term care experience preferred
Knowledge/Skills/Abilities:
oExperience or specialty training required in vital signs, bed making and resident transfer techniquesoStrong computer skills to complete patient data entry and notesoIn compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferredCertifications/Licensure:

Essential Functions1.
Provide consistently exceptional care at all times.

As directed by licensed nursing & therapy staff, provides care in a manner that is safe and sensitive to the rights of each resident.

Performs hourly rounding on all assigned residents consistently on every shift4.
As directed, assists the care team by collecting resident care data at designated and appropriate intervals on all assigned residents.
Reports finding to licensed staff.

Recognizes and accurately reports relevant resident information to licensed staff.
oReports on all significant changes in resident condition or motivational level.
oNotifies charge nurse of feeding problems, reassessment needs.
oNotifies PT and charge nurse of any mobility problems, referral or reassessment needs.
6.
Utilizes time, equipment, and supplies in an efficient & safe manner.
7.
Completes all assigned duties during designated shift.
Prioritizes resident care activities based on care needs.
Recognizes need for purpose vs.
task when caring for residents.

Provides personal care assistance to residents, encouraging self care and independence but offering and providing help as needed:
oPersonal hygiene - bathing, bathroom/bedpan, oral care, AM and PMcareoPositioningoDressingoAmbulation & transfersoEatingoTransport9.
Under direction of the therapist and charge nurse, performs routine physical therapy treatmentsoRange of MotionoStrengthening ExercisesoEnsures placement of restorative devices10.
Performs basic procedures & care to including:
oApplication of simple dressings within CNA scope of practiceoSpecimen collectionoReinforces team management of bowel/bladder programoAssists residents with nutritional intake.

Communicates appropriate information regarding resident safety to charge RN12.
Follows all appropriate infection prevention measures in place in the SNF at all times.
13.
Ensures that all aspects of resident care are documented.
Documentation is complete, accurate and timely.
14.
Supplies and EquipmentoOperates equipment and ensures proper functioning of equipmentoFollows procedure when equipment malfunctions.
oEnsures use of supplies are accurately documented15.
Demonstrates knowledge and skills necessary to provide care appropriate to the age of the residents served.
16.
Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards17.
Adheres to all hospital and departmental policies regarding attendance and punctuality.
18.
Responds to the needs of the department by performing other duties, as necessary.
