POSITION SUMMARY
The care coordinator works within the context of a primary care medical home, from a team approach, and in continuous partnership with families and providers to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of child and family well-being. This position provides professional nursing care utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on ages of the population served (neonate through geriatric-65 years or older) and in collaboration with the multidisciplinary team. Knowledge and skills necessary to identify and meet the individualized needs of all patients served is demonstrated.
POSITION REQUIREMENTS
Minimum Education:
Completion of an accredited nursing program.
Completion of a Medical Home/Managed care program
Current Registered Nurse license issues by the State of California, or ability to obtain prior to start date.
Current Basic Life Support for Healthcare Providers certification from the American Heart Association, or ability to obtain prior to start date.
Minimum
Experience:
At least one year of relevant experience within the last three years in an outpatient clinic or acute care setting.
Sufficient computer skills as are required to complete an online application and pre-employment/annual online learning requirements, as well as to enter information into the electronic medical records system.
Ability to effectively communicate in English, in compliance with patient safety standards. Bi-lingual abilities preferred.
Essential leadership, advocacy, communication, education and counseling, and resource research skills

Culturally effective capabilities demonstrating a sensitivity and responsiveness to varying cultural characteristics and beliefs
PHYSICAL DEMANDS:
Prolonged, extensive or considerable standing, walking and sitting.
Considerable reaching, stooping, bending, kneeling, crouching.
Lifts, positions, pushes or transfers patients.
Must be able to lift at least 50 lbs.
