The Case Manager RN provides individual patient care coordination. Healthcare services are coordinated on behalf of the patient to promote care delivery in the appropriate level of care which will optimize clinical outcomes and customer service. The Case Manager is responsible for facilitating and expediting care delivered to patients during their entire episode. This process requires the case manager to have direct contact with the patient, the patient's family, the patient's physicians and appropriate health care team members to ensure care coordination. Services are provided based on the ages of the population served; Newborn through Geriatric - 65 years or older and in collaboration with the multidisciplinary care team.


Minimum Education
Completion of an accredited Nursing program
Bachelor's degree preferred
Current California RN license or ability to obtain prior to hire date
Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date

Minimum Experience
3 years of experience in the last 10 years in an acute hospital setting
Recent experience as an acute hospital or managed care case manager preferred
Previous computer experience and data base management desirable
Sufficient computer skills are required to complete an online application and the pre-employment annual training requirements
In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred

Physical Demands
Prolonged, extensive or considerable standing/walking/sitting
Considerable reaching, stooping, bending, kneeling, crouching
Lifts, positions, pushes and/or transfer patients
Must be able to lift a minimum of 50 lbs
Exposure to stressful environments that may include communicating with difficult individuals in emotional situations
Contact with patients and guests under a wide variety of circumstances
Regularly exposed to the risk of blood borne diseases
Exposure to infections and contagious diseases
Subject to varying and unpredictable situations, including the handling of emergency or crisis situations
Subject to pressure due to irregular hours, frequent interruptions and stressful situations due to multiple demands

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
